Complete the entire assessment, not just for the three priority areas that you select. Some sections may be N/A if the person did not require any resources in the area throughout the story.

Please keep in mind that in a typical community setting, you would be including the person’s own opinion of their current resource status, as well as their self-selected priority areas. Since this is a fictional project, however, please speculate about what resource areas you think are most important, based on what you heard in the recordings and your knowledge of community resources as well as the goals, values, and principles of Psychiatric Rehabilitation.

Part 2: Imagine that you have just been hired at the same agency as in the Fly on the Wall Project, and you will be taking over the case management responsibilities for the person served you chose in Part 1 from LueAnn or Derek.
Write a 3-4 page narrative about how you would continue to assist the person with their community resource needs over the next 3 months, paying specific attention to the top 3 Priority Areas you noted above.
Make sure to incorporate content from the course in your plans, and include considerations about the six functions of the case manager (below) when describing the services you intend to offer to the person you choose to work with.
Case Management Functions
1. Assessing: Identifying all of the things that a person wants and needs to live successfully in the community. 2. Planning: Developing a comprehensive service and support plan with the person (i.e., specifying how he or she will meet his needs/wants). 3. Linking: Connecting the individual with appropriate service providers or other community resource agencies. 4. Monitoring: Ensuring that quality services and supports are actually delivered to the individual. 5. Advocating: Acting to resolve any problems related to service provision and resource acquisition. 6. Evaluating: Assessing the outcomes of service and support provision and proving follow-up support and/or services. |
